re: poll
If the president were to die the chairperson of the parliament becomes the acting president until a new president is elected; the acting president will serve the rest of the term. The acting president is not given the authority to address the nation or parliament, dismiss the legislative branch, appoint candidates for parliamentary approval of government or judicial posts. The acting president cannot call for a referendum, grant military ranks, state orders or exercise their right of pardon. There are no constitutional provisions for presidential succession in case both the president and chairman's posts are vacant.
If the prime minister were to die the Deputy Prime minister would serve as acting PM until the president nominates a candidate for prime minister. Once the parliament confirms the nominee the new PM has the ability to form a new cabinet or not.

If the president dies in office, the vice president takes over until the next election. If the prime minister dies, an election is held after at least a week.

Re: Poll
We don't have a head of state
Since the Executive council is elected by Parliament, they would just elect a new one. If only the Speaker of the Executive council would die, his seat would likewise be filled by a vote in Parliament, the council would then elect a new speaker from its midst

Re : poll
If the monarch dies, the current crown prince/crown princess would take over but if the crown prince/crown princess is too young to be a monarch, the former queen/king consort would take over until the crown prince/crown princess is old enough to be a monarch.
About the prime minister, the parliament would take over the prime ministry office temporarily and later they would held a discussion on who is going to be the prime minister and elect a new prime minister later
However in wartime, the current minister of defense would take over the prime minister post.

The situation of a Chancellor dying is a little bit weird for the line of succession
The role is passed from the late Chancellor to the Prince of Ancurych (one of the Cartasan Counties) until the 34th day of the month, whence at the edge of Egreilden (The largest Cartasan Forest) all the candidates enter at 1Pm and first to bring the national animal out of the woods, both themselves and the creature unharmed, is Chancellor until the next election
Though Egreilden can be very dangerous as the national animals only habitat is inside it
Because Revolution and stuff again
